Patient Preparation     If a cholesterol measurement is to be performed along with Triglycerides, but not part of a lipid panel, then the patient should be fasting 9-12 hours prior to collection. If the cholesterol is ordered as part of a lipid panel, then a fasting sample is not required.
The assay manufacturer Beckman Coulter advises: "N-Acetyl Cysteine (NAC), when administered in therapeutic concentrations (for the treatment of acetaminophen overdose), has been. . . determined to interfere with assays for. . . cholesterol, uric acid" where "NAC interference may lead to falsely low results." According to Beckman Coulter, the NAC interference should be insignificant by 12 hours after completion of the initial loading dose of an IV infusion treatment regimen consisting of an initial loading dose of 150 mg/kg administered over 1 hour, a second dose of 50 mg/kg administered over 4 hrs and a third dose of 100 mg/kg administered over 16 hrs.

​It is reported that chronic insulin resistance is also associated with various types of cancer such as colorectal cancer, pancreatic cancer, endometrial cancer, and breast cancer.
Insulin resistance is when cells in your muscles, fat, and liver don't respond well to insulin and can't use glucose from your blood for energy. To make up for it, your pancreas makes more insulin. Over time, your blood sugar levels go up.
Some signs of insulin resistance include:
  • A waistline over 40 inches in men and 35 inches in women
  • Blood pressure readings of 130/80 or higher
  • A fasting glucose level over 100 mg/dL
  • A fasting triglyceride level over 150 mg/dL

Effective Treatment for Insulin Resistance
Carbohydrates such as simple sugars, grains and starchy vegetables should be avoided as they stimulate insulin secretion. They should be replaced with higher protein containing foods and non starchy vegetables.
Adequate vitamin D intake
Aerobic and resistance training--High-Intensity Interval Training (HIIT) has been found to be superior to low intensity cardio. Of course I suggest that people completely out of shape to work their way up to HIIT. Start out slow. Some exercise is better than none.
Get adequate sleep.
Reduce stress-- meditation is recommended.
Get adequate magnesium intake.
Take Berberine supplementation.

HLA Typing for Celiac Disease Test Code: 17135
Clinical Use Rule out celiac disease (CD) diagnosis Determine genetic susceptibility to CD Clinical Background CD is caused by an immune response to gluten.1,2 Patients with CD may be asymptomatic, but symptoms can include diarrhea and abdominal pain or discomfort in adults; depending on age, children can present with these symptoms plus others (eg, failure to thrive, short stature, delayed puberty). An initial diagnosis of CD may be based on a clinical history, duodenal biopsy and histology (eg, villous atrophy), and serology (eg, positive anti-tissue transglutaminase [TTG] antibody). In certain clinical settings, as described below, human leukocyte antigen (HLA) typing can be helpful in ruling out CD. HLA-DQ2/DQ8-bound gluten is recognized by T-cells and leads to an inflammatory response. The presence of HLA-DQ2 (DQA1*05/DQB1*02) and DQ8 (DQA1*03/DQB1*0302) is the most important genetic risk factor for CD.3 Approximately 95% of patients with CD have HLA-DQ2 alleles and 5% have HLA-DQ8 alleles.3,4 However, these alleles are also present in 25% to 40% of the US population, though only about 1% develop CD.3 Thus, detection of either allele by HLA typing does not necessarily indicate CD, and HLA typing is not recommended as part of the initial diagnostic evaluation of patients with suspected CD.3,5 However, because CD is extremely rare in patients who do not have HLA-DQ2 or HLA-DQ8, negative results for these alleles provide strong evidence that the patient does not have CD. Given the high negative predictive value of HLA typing for CD, the American College of Gastroenterologists (ACG) recommends HLA typing to rule out a diagnosis of CD in certain clinical settings in which other diagnostics approaches may not be appropriate or have yielded uncertain results.3 These settings include inconsistent serology and histology test results (eg, positive serology, normal histology), equivocal small-bowel histological findings in seronegative patients, and Down syndrome.3,5 HLA typing is also recommended for some patients on a gluten-free diet which can lead to normal serology and histology results. Specifically, HLA typing is recommended for patients on a gluten-free diet if they are being considered for gluten challenge, have not been tested for CD before starting a gluten-free diet, or have suspected refractory CD (persistent symptoms after 6-12 months on a gluten-free diet) and a questionable original diagnosis of CD.3 Individuals Suitable for Testing Individuals with discrepant celiac-specific serology and histology findings Seronegative individuals with equivocal small-bowel histological results Individuals with Down syndrome Individuals on a gluten-free diet who are being considered for gluten challenge, were not tested for CD before starting a gluten-free diet, or may have suspected refractory CD Method Polymerase chain reaction (PCR) amplification of extracted DNA followed by hybridization with sequence-specific oligonucleotide probes. Results reported: positive or negative for HLA-DQ2 and -DQ8; specific DQA1 and DQB1 variants detected (to determine zygosity); patient-specific interpretation Interpretive Information Negative results for both HLA-DQ2 and HLA-DQ8 virtually exclude the diagnosis of CD and indicate an extremely low risk for subsequent development of the disease (≥95% negative predictive value).6 In rare cases, however, when DQ2 and DQ8 heterodimers are negative, the presence of 1 of the DQ2 alleles (eg, either DQA1*05 or DQB1*02) is consistent with a diagnosis of CD.1 A positive DQ2 or DQ8 result can be consistent with a CD diagnosis, but it does not indicate a CD diagnosis. The risk for developing CD in individuals with positive DQ2 or DQ8 results varies depending on the alleles present, with the highest risk in homozygotes for DQ2.7,8 DQB1*0201 results may reflect the severity of disease. Although data are conflicting, homozygosity for this allele has been associated with having more severe villous atrophy, a slower rate of villous recovery once a gluten-free diet has been instituted, and developing CD at a younger age.9
